Appendix

The Appendix may only contain materials specifically allowed by NIH application instructions or the Notice of Funding Opportunity (NOFO). Applicants should not use the Appendix to provide additional information that expands upon the application or circumvents page limits.

Include:

The only allowable Appendix materials are:

  • Blank data collection forms, survey instruments, questionnaires, or screenshots of these materials.
  • Simple lists of interview questions.
  • Blank informed consent and assent forms.
  • Other materials only when explicitly permitted in the NOFO.

When submitting blank forms or question lists, do not include:

  • Data or data compilations
  • Lists of variables, abbreviations, or acronyms
  • Data analyses
  • Publications or manuscripts
  • Manuals or instructions
  • Descriptions, figures, diagrams, or drawings of methods or equipment

Writing Guidance:

Use descriptive file names for all Appendix attachments. A summary sheet listing all Appendix materials is encouraged, but not required. If provided, include the summary sheet as the first Appendix attachment.

Refer to the NOFO to determine whether special Appendix instructions apply to your application, as requirements may vary by funding opportunity.


Important Notes:

  • Hyperlinks and URLs are not allowed unless specifically permitted by the NOFO.
  • A maximum of 10 PDF attachments may be included in the Appendix.
    • If more than 10 attachments are needed, combine the remaining materials into the tenth attachment.
  • Tables other than required Training Grant Data Tables must be included in the Program Plan and should not be placed in the Appendix.
  • Information that expands upon or complements content elsewhere in the application is not allowed unless specifically permitted as Appendix material.
  • Material Transfer Agreements (MTAs) should not be included unless specifically allowed by the NOFO.
  • No materials other than those specifically permitted may be included.
  • Relocating disallowed materials to another section of the application may still result in a noncompliant application.
  • Applications that do not follow NIH or NOFO Appendix requirements may be withdrawn and not reviewed.
